Filling Methadone Prescriptions

Select the appropriate tab to learn about filling methadone prescriptions in your province.

Methadone prescriptions should be billed in milligrams (mg). As third parties need the quantities in milliliters (mL), Propel Rx automatically converts the quantity to mL in the background. Doses cannot exceed 1 decimal place by volume (mL). The prescription can be billed with or without carries, depending on the situation.

With the introduction of decimal dispensing in Propel Rx, you have the flexibility to fill methadone prescriptions using the methadone Drug Folder or an existing Mixture Folder. The methadone Drug can be set up to bill per milligram (mg) or milliliter (mL). The prescription can be billed with or without carries, depending on the situation.

Setting up the Drug Folder

There are three - four steps when setting up the methadone Drug Folder. If you are using a methadone mixture to fill prescriptions, you do not need to complete this section.

If you want to switch from using a methadone Mixture Folder to a Drug Folder, we recommend waiting until all refills of existing authorizations are used up and then creating a new prescription chain for the Drug.

Methadone can be billed per mg or mL in Propel Rx. The following methadone 10mg/mL DINs have mg and mL pack sizes entered:

The choice of which pack size to use depends on your preference.

To select a default pack size:

  1. Open the methadone Drug Folder.

  2. Select the Inventory tab.

  3. Select the Default checkbox for the pack size you want to use, if not already selected. Select the Monitor checkbox and enter a Re-Order Point and Re-Order # Packs if you want to track inventory for this Drug in Propel Rx.

    The description, "use to bill per mg," has been added to the mg pack sizes for easier identification.

  4. Select Save.

After upgrading to Propel Rx, you must set the Dispensing Unit of Measure in the methadone Drug Folder. Once set and prescriptions have been dispensed using the pack size, the field is not editable. A prompt appears if you attempt to fill a methadone prescription before setting the Dispensing Unit of Measure.

This is not required if you use a methadone Mixture Folder for filling.

  1. In the Drug Folder Inventory tab, dropdown the Dispensing Unit of Measure and select the appropriate unit for each pack size.

    Select Milligrams if the description for the pack size is "use to bill per mg." Select Milliliters if the pack size reflects the actual volume of the bottle.

  2. Select Save.

If you select a mg pack size to fill methadone prescriptions, you may need to enter a TP Rule to convert the adjudicated quantity to mL, if required by the third party.

The TP Rule for Ontario Drug Benefit (ODB) has been entered automatically for the DINs in Table 1. Methadone 10mg/mL DINs with mg and mL pack sizes.

  1. Select the Drug Folder main tab.

  2. Select TP Rules.

  3. Select Add.

  4. Select the Third Party and TP Plan that requires methadone to be adjudicated in mL.

  5. Select the mg Pack Size used for filling prescriptions.

  6. In the Rx Qty field, enter 10. In the TP Qty field, enter 1.

  7. Select OK.

  8. Select Save.

The Methadone Custom Class is used to generate the Potential Missed Doses / Billings report and set specific Pricing Strategies. When billing to ODB, the unit price submitted depends on whether methadone is billed by mg or mL. The Prov $ or Provincial Price in the Drug Folder is set up for billing by mL. If you want to bill by mg, you need to use the Supplier Price for ODB claims.

  1. In the Drug Folder Alternative tab, select Add.

  2. Select Drug Benefit-All as the Third Party Plan and Methadone as the Custom Class.

  3. Select Save.

  4. Select > Pricing.

  5. Confirm that the Methadone Custom Class appears under Drug Benefit. If not present, select Drug Benefit and select Add. In the Create New Strategy window, select Methadone in the Custom Class field. Select OK.

  6. In the Strategies tab for the Methadone Custom Class, select the following Cost Method depending on how you bill methadone:

    • ELI/ACQ/SUP or ELI/SUP/ACQ if billing by mL

    • SUP/ACQ/ELI if billing by mg

      If billing by mg, you may need to waive a portion of the cost or markup in the Claim Summary window after adjudicating to ODB. This is because the Supplier Price may differ slightly from the Provincial Price.

  7. Confirm the fee and markup for the Methadone Custom Class are appropriate for ODB.

    For ODB, methadone claims are reimbursed with the same fee and markup as regular claims.

  8. Select Save.

Filling a Methadone Prescription

Methadone can be filled for pain or addiction management. For more information on the different scenarios, select the dropdowns below.

A methadone prescription without carries is filled using the same steps as a regular narcotic prescription, except you can select the ingestion date which prints on the vial label and prescription file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone in one dose.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG (include daily dose in mL and mg)

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. The ingestion date defaults to the current date. If the date is not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  7. Select OK.

  8. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    ODB requires the TP Qty to be in mL.

  9. Select Fill.

A methadone prescription with carries is filled similarly to one without carries. Each dose is filled as a prescription.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone in one dose.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG (include daily dose in mL and mg)

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail. Enter the # of Carries and press Tab on the keyboard.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  7. The ingestion dates start from the current date. One day is highlighted for each methadone prescription. If you need to modify the ingestion dates, select Clear Days and manually highlight the correct days.

    The Even Days and Odd Days highlight every other day from the first Ingestion Date.

  8. Select OK. The witnessed dose and carries are filled automatically. Each carry is filled as a separate prescription.

    For ODB claims, Propel Rx enters an UA intervention code for each claim, except the first.

The Replacement Dose checkbox in Rx Detail can be used to indicate a prescription is a replacement dose. A '*REPL*' indicator appears in the Audit History for the prescription. A replacement dose may be required in a situation such as when the patient spills the medication.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone in the replacement dose.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG (include daily dose in mL and mg)

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Select the Replacement Dose checkbox.

  7. The ingestion date defaults to the current date. If the date is not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  8. Select OK.

  9. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    ODB requires the TP Qty to be in mL.

  10. Select Fill.

This section is only applicable if using methadone 10mg/mL for pain management.

The Pain Management checkbox in Rx Detail can be used to indicate a prescription is for pain management. When the checkbox is selected, the Ingestion Date calendar and # of Carries field are disabled in the Methadone HCL window. 'PAIN MANAGEMENT' is indicated on the file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone being dispensed.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Select the Pain Management checkbox.

  7. Select OK.

  8. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

  9. Select Fill.

  7. Repeat steps 3 - 6 to add additional third parties that require adjudication in mL.

  8. Select OK.

  9. Select Save.

If the date is not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail and the Total # Doses. The ML Per Dose cannot be more than 1 decimal place.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  8. Select OK.

  9. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    Alberta Blue Cross requires the TP Qty to be in mL.

  10. Select Fill.

    In Technical Validation and Clinical Review, the Total Dose displayed reflects the quantity entered in Rx Detail and not the quantity for each dose in a multiple daily dosing scenario.

A methadone prescription without take-home doses is filled using the same steps as a regular narcotic prescription, except you can select the ingestion date which prints on the vial label and prescription file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

The choice of which pack size to use depends on your preference.

To select a default pack size:

  1. Open the methadone Drug Folder.

  2. Select the Inventory tab.

  3. Select the Default checkbox for the pack size you want to use, if not already selected. Select the Monitor checkbox and enter a Re-Order Point and Re-Order # Packs if you want to track inventory for this Drug in Propel Rx.

    The description, "use to bill per mg," has been added to the mg pack sizes for easier identification.

  4. Select Save.

After upgrading to Propel Rx, you must set the Dispensing Unit of Measure in the methadone Drug Folder. Once set and prescriptions have been dispensed using the pack size, the field is not editable. A prompt appears if you attempt to fill a methadone prescription before setting the Dispensing Unit of Measure.

This is not required if you use a methadone Mixture Folder for filling.

  1. In the Drug Folder Inventory tab, dropdown the Dispensing Unit of Measure and select the appropriate unit for each pack size.

    Select Milligrams if the description for the pack size is "use to bill per mg." Select Milliliters if the pack size reflects the actual volume of the bottle.

  2. Select Save.

If you select a mg pack size to fill methadone prescriptions, you need to enter a TP Rule to convert the adjudicated quantity to mL for Manitoba Health and other third parties, if required.

In Rx Detail, the Actual Qty field can be used to enter a different quantity to submit to third parties. This field can be used in lieu of TP Rules but the quantity entered submits to all third parties in the Rx Detail Third Party tab.

  1. Select the Drug Folder main tab.

  2. Select TP Rules.

  3. Select Add.

  4. In the Third Party field, select Manitoba Health.

  5. Select the mg Pack Size used for filling prescriptions.

  6. In the Rx Qty field, enter 10. In the TP Qty field, enter 1.

  7. Repeat steps 3 - 6 to add additional third parties that require adjudication in mL.

  8. Select OK.

  9. Select Save.

The Methadone Custom Class is needed to generate the Potential Missed Doses / Billings report and set up specific Pricing Strategies. When billing to Manitoba Health, the unit price submitted depends on whether methadone is billed by mg or mL. The Prov $ or Provincial Price in the Drug Folder is set up for billing by mL. If you want to bill by mg, you need to use the Supplier Price for Manitoba Health claims.

  1. In the Drug Folder Alternative tab, select Add.

  2. Select Manitoba Health-ALL as the Third Party Plan and Methadone as the Custom Class.

  3. Select Save.

  4. Select > Pricing.

  5. Confirm that the Methadone Custom Class appears under Manitoba Health. If not present, select Manitoba Health and select Add. In the Create New Strategy window, select Methadone in the Custom Class field. Select OK.

  6. In the Strategies tab for the Methadone Custom Class, select the following Cost Method depending on how you bill methadone:

    • ELI/ACQ/SUP or ELI/SUP/ACQ if billing by mL

    • SUP/ACQ/ELI if billing by mg

      If billing by mg, you may need to waive a portion of the cost or markup in the Claim Summary window after adjudicating to Manitoba Health. This is because the Supplier Price may differ slightly from the Provincial Price.

  7. Confirm the fee for the Methadone Custom Class are appropriate for Manitoba Health.

  8. Select Save.

Filling a Methadone Prescription

Methadone can be filled for pain or addiction management. For more information on the different scenarios, select the dropdowns below.

A methadone prescription without take-home doses is filled using the same steps as a regular narcotic prescription, except you can select the ingestion date which prints on the vial label and prescription file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the total quantity of methadone being dispensed to the patient.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Enter the number of doses being dispensed in the # of Doses field.

  7. The ingestion date defaults to the current date. If the date is not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail and the # of Doses.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  8. Select OK.

  9. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    Manitoba Health requires the TP Qty to be in mL.

    The Rx Detail Actual Qty field can be used to submit a different quantity to all third parties in the Bill Code field. If you need to override the Actual Qty for a third party, select the Third Party tab and change the TP Qty.

A methadone prescription with take-home doses is filled similarly to one without take-home doses. The witnessed and take-home doses are filled as a single prescription.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the total quantity of methadone (witnessed + take home doses) being dispensed to the patient.

  4. Enter the remaining prescription information.

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS = total day supply for the witnessed + take-home doses

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Enter the number of doses being dispensed in the # of Doses field.

  7. The ingestion dates start from the current date. If the dates are not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail and the # of Doses.

    In Technical Validation and Clinical Review, the Total Dose displayed reflects the total quantity filled in the prescription and not the quantity for each dose.

The Replacement Dose checkbox in Rx Detail can be used to indicate a prescription is a replacement dose. A '*REPL*' indicator appears in the Audit History for the prescription. A replacement dose may be required in a situation such as when the patient spills the medication.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone in the replacement dose.

  4. Enter the remaining prescription information including:

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. In the # of Doses field, enter 1.

  7. Select the Replacement Dose checkbox.

  8. The ingestion date defaults to the current date. If the date is not correct, select Clear Days and manually highlight the correct date.

  9. Select OK.

The Pain Management checkbox in Rx Detail can be used to indicate a prescription is for pain management. When the checkbox is selected, the Ingestion Date calendar and # of Doses field are disabled in the Methadone HCL window. 'PAIN MANAGEMENT' is indicated on the file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone being dispensed.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Select the Pain Management checkbox.

  7. Select OK.

Methadone prescriptions should be billed in milligrams (mg). As third parties need the quantities in milliliters (mL), Propel Rx automatically converts the quantity to mL in the background. Doses cannot exceed 1 decimal place by volume (mL). The prescription can be billed with or without take-home doses, depending on the situation.

With the introduction of decimal dispensing in Propel Rx, you have the flexibility to fill methadone prescriptions using the methadone Drug Folder or an existing Mixture Folder. The methadone Drug can be set up to bill per milligram (mg) or milliliter (mL). The prescription can be billed with or without take-home doses, depending on the situation.

Setting up the Drug Folder

There are three - four steps when setting up the methadone Drug Folder. If you are using a methadone mixture to fill prescriptions, you do not need to complete this section.

If you want to switch from using a methadone Mixture Folder to a Drug Folder, we recommend waiting until all refills of existing authorizations are used up and then creating a new prescription chain for the Drug.

Methadone can be billed per mg or mL in Propel Rx. The following methadone 10mg/mL DINs have mg and mL pack sizes entered.

  2. If the prescription is new, enter the number of times per week the pharmacist witnessed the patient's methadone ingestion in the QA and Qty fields. A maximum of 7 can be entered.

  3. Enter the remaining prescription information including:

    • Patient

    • Prescriber = pharmacist

    • Drug = 951326 (on-site) or 951327 (off-site)

    • SIG

    • DS = 7

    • V/W

    • Ready time

  4. In Rx Detail, select Fill.

A methadone prescription without take-home doses is filled using the same steps as a regular narcotic prescription, except you can select the ingestion date which prints on the vial label and prescription file copy. An Alternative DIN can be entered and locked for submission to PharmaCare and Non-Insured Health Benefits.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the total quantity of methadone being dispensed to the patient.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Enter the number of doses being dispensed in the # of Doses field.

  7. Enter the applicable PharmaCare OAT PIN in the Alternative DIN field. Select the Lock checkbox if you want the Alternative DIN to carry over for refills.

    The Alternative DIN field is used for PharmaCare and Non-Insured Health Benefits claims.

  8. The ingestion date defaults to the current date. If the date is not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail and the # of Doses.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

  9. Select OK.

  10. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    PharmaCare requires the TP Qty to be in mL.

    The Rx Detail Actual Qty field can be used to submit a different quantity to all third parties in the Bill Code field. If you need to override the Actual Qty for a third party, select the Third Party tab and change the TP Qty.

  11. Select Fill.

A methadone prescription with take-home doses is filled similarly to one without take-home doses. The witnessed and take-home doses are filled as a single prescription.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the total quantity of methadone (witnessed + take home doses) being dispensed to the patient.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS = total days supply for the witnessed + take-home doses

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Enter the number of doses being dispensed in the # of Doses field.

  7. Enter the applicable PharmaCare OAT PIN in the Alternative DIN field. Select the Lock checkbox if you want the Alternative DIN to carry over for refills.

    The Alternative DIN field is used for PharmaCare and Non-Insured Health Benefits claims.

  8. The ingestion dates start from the current date. If the dates are not correct, select Clear Days and manually highlight the correct date. The MG Per Dose or ML Per Dose is calculated based on the Qty entered in Rx Detail and the # of Doses.

    For methadone mixture prescriptions, Total Dose (MG) is equivalent to MG Per Dose and Total Dose (ML) is equivalent to ML Per Dose.

    The Even Days and Odd Days highlight every other day from the first Ingestion Date.

  9. Select OK.

  10. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    PharmaCare requires the TP Qty to be in mL.

  11. Select Fill.

  12. A single prescription is filled for all doses.

    In Technical Validation and Clinical Review, the Total Dose displayed reflects the total quantity filled in the prescription and not the quantity for each dose.

The Replacement Dose checkbox in Rx Detail can be used to indicate a prescription is a replacement dose. A '*REPL*' indicator appears in the Audit History for the prescription. A replacement dose may be required in a situation such as when the patient spills the medication.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone in the replacement dose.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. In the # of Doses field, enter 1.

  7. Select the Replacement Dose checkbox.

  8. Enter the applicable PharmaCare OAT PIN in the Alternative DIN field. Select the Lock checkbox if you want the Alternative DIN to carry over for refills.

    The Alternative DIN field is used for PharmaCare and Non-Insured Health Benefits claims.

  11. If billing per mg and adjudicating to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

  12. Select Fill.

This section is only applicable if using methadone 10mg/mL for pain management.

The Pain Management checkbox in Rx Detail can be used to indicate a prescription is for pain management. When the checkbox is selected, the Ingestion Date calendar and # of Doses field are disabled in the Methadone HCL window. 'PAIN MANAGEMENT' is indicated on the file copy.

  1. Select Intake from the Workbench or Patient Profile to create a new prescription or Refill an existing one on the Profile.

  2. If the prescription is new, enter the total quantity of methadone authorized in the QA field.

  3. In the Qty field, enter the quantity of methadone being dispensed.

  4. Enter the remaining prescription information including:

    • Patient

    • Doctor

    • Drug

    • SIG

    • DS

    • V/W

    • Ready time

  5. In Rx Detail, select Rx > Methadone HCL.

  6. Select the Pain Management checkbox.

    Do not enter an OAT PIN in the Alternative DIN field.

  7. Select OK.

  8. If billing to a third party, select the Rx Detail Third Party tab. Confirm the TP Qty is displaying in mL, if required by the third party.

    The Rx Detail Actual Qty field can be used to submit a different quantity to all third parties in the Bill Code field. If you need to override the Actual Qty for a third party, select the Third Party tab and change the TP Qty.
